- Flyer Template—Use to assist with the creation of adult participant recruitment fliers to be shared in public
- Email Template—Use to assist with the creation of adult participant recruitment messages to be sent via email
- Adult Script Template—Use to assist with the creation of a participant recruitment script to be used for in-person recruitment of adults
Disclosure Templates

Adult Participant Populations (ages 18+)
- Informed Consent Template—Use to support the drafting of an informed consent form for adults
- Anonymous Survey Informed Consent Template—Use to support the drafting of an informed consent form for survey research involving adults
Adolescent & Children Participant Populations (ages under 18)
- Children Informed Assent Template—Use to support the drafting of an informed assent form to children ages 11-17
- Note: Language should be modified, as appropriate to the population of children
- Parent/Guardian Consent for Child Participation—Use to support the drafting of an informed consent form for the legal guardians of children invited to participate in a study
- Note: This form should be accompanied by a children’s assent form
- Sample Assent Statement—Use to support the drafting of a script/form to obtain a child’s agreement to participate in the research study
